Request a Health Needs Assessment

Should you wish to request a specific topic for a Health Needs Assessment (HNA), please read our privacy notice and then complete this form.

All requests will be reviewed by the Population Health Hub Steering Group which meets monthly.

The usual timescales for completing a needs assessment are as follows (not including primary data collection, such as focus groups or surveys):

• Rapid Assessment: 2 to 3 months
• Detailed Assessment: 6 months

Upcoming needs assessments can be programmed up to 1 year in advance, please submit requests at the earliest opportunity.
